Attached is the parent letter, Rank One instructions and the consent form. A copy of the physical form will be available in the gym where physicals are being conducted. Parents only need to sign the consent form.

Please note: The medical history part of the physical has been uploaded into Rank One where parents can complete it with all other ​forms.
