1. Immunization Records
Fully completed immunization records. Oklahoma state law requires parents or guardians to provide proof of current immunizations before a student can enter or attend classes. If you choose to opt your child out of one or more required vaccines, you must obtain an official state-approved exemption certificate.

2. Immunization Exemption Certificate
The Oklahoma State Department of Health Immunization Service requires all exemption requests to be processed online. Parents/guardians seeking an immunization exemption due to medical conditions, religious concerns, or personal beliefs must submit a request through the Oklahoma State Department of Health's online portal. To begin, visit the OSDH Immunizations Page to obtain instructions and access the portal. (Schools can no longer accept hand-written notes or blank forms directly from parents)
Once submitted, the OSDH Immunization Service will review your request and provide a decision within 1-3 business days. If approved, you will receive an email notification prompting you to log back into the portal to download the approved exemption certificate. It is the responsibility of the parent/guardian to provide the approved certificate to the Enrollment Center.
If the exemption request is denied, all immunization requirements must be fulfilled before enrollment can proceed.
How to Request an Immunization Exemption:
- Create an Account: Visit the official OSDH Immunization Portal to set up a parent profile.
- Submit Online: Complete and submit your request through the secure portal. You may select a Medical, Religious, or Personal belief exemption.
- Note: Medical exemptions require a signed document from a licensed physician.
- Approval Window: The Oklahoma State Department of Health will review your application. Approvals typically take 1 to 3 business days.
- Download and Print: Once approved, you will receive an email notification. Log back into the portal to download your official Certificate of Exemption.
- Submit to the School: Print the approved certificate and turn it in to the school office along with your other enrollment paperwork.
Important Rules to Remember:
- 6th Grade Expiration: All approved vaccine exemptions expire at the end of a student’s 6th-grade year. Before a student enters 7th grade, a new immunization exemption must be requested from the Oklahoma State Department of Health. Previous exemptions do not automatically carry over and must be renewed to remain valid.
- Please note that exemption applications for 7th grade will not be accepted by the Oklahoma State Department of Health until after the student’s last day of 6th grade.To ensure a smooth transition into 7th grade, we encourage parents to plan accordingly and submit exemption requests as soon as their student has completed 6th grade. - Outbreak Exclusions: In the event of a vaccine-preventable disease outbreak at our facility, unimmunized students may be legally excluded from attending school for the duration of the outbreak for their safety.
